Роль самоуправления в реабилитации пациентов с сахарным диабетом 2-го типа

Аннотация

Актуальность. Стремительный рост распространения сахарного диабета 2-го типа (СД2) становится глобальной мировой проблемой. Неотъемлемой частью ведения пациентов с СД2 является реабилитация – комплекс действий, направленных на улучшение качества жизни и профилактику осложнений. В последние годы на место концепции, в которой врач играет ведущую роль, приходит принцип партнерства пациента и врача. Такая модель предусматривает способность больного к самоуправлению (англ. self-management) в процессе реабилитации. В связи с этим актуальным представляется изучение факторов, влияющих на эффективность самоуправления.

Цель: выявить ключевые аспекты реабилитации пациентов с СД2, рассмотреть факторы, препятствующие и способствующие успешной организации самоуправления.

Материал и методы. Поиск публикаций проведен в научных базах данных и электронных библиотеках: PubMed/ MEDLINE, Google Scholar, eLibrary. В общей сложности найдено 3694 публикации, из которых 70 полностью соответствовали критериям отбора и были включены в обзор. Поиск и отбор источников выполнен с учетом рекомендаций PRISMA.

Результаты. Основными аспектами реабилитации пациентов с СД2 являются постоянный гликемический контроль, коррекция питания, регулярное выполнение физических упражнений, адекватный уход за стопами. Огромное влияние на эффективность реабилитации оказывает способность пациента к самостоятельному управлению этими процессами. Ему препятствуют такие факторы, как диабетический дистресс, усталость, коморбидные заболевания, отсутствие доступа к качественной медицинской помощи. Успешное самоуправление реабилитацией связано с социальной, прежде всего семейной поддержкой пациента. Также большое значение имеет достаточный уровень осведомленности о СД2, позволяющий пациенту грамотно выстраивать процесс реабилитации. Значительную помощь в организации самоуправления оказывают современные технологии, в частности специализированные мобильные приложения для пациентов с СД2.

Заключение. Успешная реабилитация при СД2 напрямую зависит от навыков самоуправления у пациента. Их развитию способствуют социальная поддержка, повышение грамотности в сфере здоровья и современные цифровые технологии.
