Распространенность ПКС среди лиц, перенесших COVID-19, варьируется от 3% до 12%, при этом описано более 200 симптомов, затрагивающих практически все системы органов. Целью данного обзора являлась систематизация современных подходов к реабилитации пациентов с ПКС с акцентом на мультидисциплинарные и персонализированные стратегии. Представлены краткие сведения о патофизиологии заболевания, включая персистенцию вируса, иммунную дисрегуляцию, эндотелиальную дисфункцию и нарушения вегетативной нервной системы. Рассмотрены подходы к реабилитации с позиции мультидисциплинарного ведения, интеграции принципов человеко-факторной инженерии и концепции «излечимых признаков». Затронуты такие реабилитационные методы, как физическая реабилитация с применением стратегии Pacing, респираторная и когнитивная реабилитация, психологическая поддержка, коррекция вегетативных нарушений, телереабилитация, профессиональная реабилитация и нутритивная поддержка. Поиск литературы выполнен в базах PubMed/MEDLINE, Scopus, Web of Science, Cochrane Library и eLibrary за период с января 2020 г. по март 2026 г. с использованием ключевых слов и их комбинаций: “post-COVID-19 condition”, “long COVID”, «постковидный синдром», “rehabilitation”, «реабилитация», “multidisciplinary approach”, “exercise therapy”, “pacing”, “cognitive rehabilitation”, “autonomic dysfunction”, “post-exertional malaise”. В обзор включены клинические рекомендации, систематические обзоры, метаанализы и рандомизированные контролируемые исследования на русском и английском языках; приоритет отдавался публикациям последних 5 лет.</p></abstract><trans-abstract xml:lang="en"><p>Post-COVID syndrome (PCS) is a multisystem condition that develops after a SARS-CoV-2 infection. More than 200 variants of PCS symptoms affect virtually all organ systems and persist for over three months. PCS prevalence among individuals who have had SARS-CoV-2 infection ranges from 3% to 12%. This review aims to systematize current approaches to PCS rehabilitation, emphasizing multidisciplinary and personalized strategies. Brief information on the pathophysiology of the condition is provided, focusing on viral persistence, immune dysregulation, endothelial dysfunction, and autonomic nervous system disorders. Rehabilitation approaches are discussed in terms of multidisciplinary management, the principles of human factors engineering, and the concept of treatable traits. Rehabilitation methods covered include physical rehabilitation using the Pacing strategy, respiratory and cognitive rehabilitation, psychological support, management of autonomic dysfunction, telerehabilitation, vocational rehabilitation, and nutritional support. A literature search was performed in the PubMed/MEDLINE, Scopus, Web of Science, Cochrane Library, and eLibrary databases for the period from January 2020 to March 2026, using the following keywords and their combinations: “post-COVID-19 condition”, “long COVID”, “post-COVID syndrome”, “rehabilitation”, “multidisciplinary approach”, “exercise therapy”, “pacing”, “cognitive rehabilitation”, “autonomic dysfunction”, and “post-exertional malaise”. Clinical guidelines, systematic reviews, meta-analyses, and randomized controlled trials published in Russian and English were included, with priority given to publications from the last five years</p></trans-abstract><kwd-group xml:lang="ru"><kwd>постковидный синдром</kwd><kwd>реабилитация</kwd><kwd>человеко-факторная инженерия</kwd><kwd>мультидисциплинарный подход</kwd></kwd-group><kwd-group xml:lang="en"><kwd>post-COVID syndrome</kwd><kwd>long COVID</kwd><kwd>rehabilitation</kwd><kwd>human factors engineering</kwd><kwd>multidisciplinary approach</kwd></kwd-group></article-meta></front><body><sec><title>ВВЕДЕНИЕ / INTRODUCTION</title><p>Пандемия COVID-19, вызванная вирусом SARS-CoV-2, привела к беспрецедентным последствиям для глобального здравоохранения. По данным Всемирной организации здравоохранения на март 2026 г., в мире зафиксировано 779 млн подтвержденных случаев (в России – 24,9 млн), и эта цифра продолжает увеличиваться [<xref ref-type="bibr" rid="cit1">1</xref>][<xref ref-type="bibr" rid="cit2">2</xref>].</p><p>По мере развития пандемии стало очевидно, что значительная часть пациентов продолжает испытывать симптомы далеко за пределами острой фазы инфекции. Это состояние, получившее название «постковидный синдром» (ПКС) или «лонг-ковид» (англ. long COVID), стало новым вызовом для систем здравоохранения во всем мире [<xref ref-type="bibr" rid="cit3">3</xref>][<xref ref-type="bibr" rid="cit4">4</xref>]. Риск развития симптомов, сохраняющихся после острого COVID-19, оценивается в 15% на каждое заражение и увеличивается при повторных инфекциях [<xref ref-type="bibr" rid="cit2">2</xref>][<xref ref-type="bibr" rid="cit5">5</xref>]. Заболевание чаще поражает женщин (примерно в 2 раза чаще мужчин) и лиц в возрасте 18–64 лет, затрагивая все этнические группы [<xref ref-type="bibr" rid="cit6">6</xref>]. ПКС нередко сопровождается повреждением широкого спектра органов и систем [<xref ref-type="bibr" rid="cit4">4</xref>][<xref ref-type="bibr" rid="cit7">7</xref>].</p><p>Распространенность ПКС варьируется от 3% до 12% в зависимости от критериев диагностики, изучаемой популяции и доминирующего штамма вируса [<xref ref-type="bibr" rid="cit4">4</xref>]. В настоящее время описано более 200 различных симптомов, затрагивающих практически все системы органов [<xref ref-type="bibr" rid="cit3">3</xref>][<xref ref-type="bibr" rid="cit4">4</xref>]. Наиболее часто регистрируемые проявления:</p><p>– конституциональные симптомы – выраженная утомляемость (астения), которая отмечается у большинства пациентов и часто носит изнуряющий характер [8–11] (утомляемость классически усиливается после физического или умственного напряжения – феномен, известный как постнагрузочное ухудшение (англ. post-exertional malaise, PEM) [<xref ref-type="bibr" rid="cit3">3</xref>][<xref ref-type="bibr" rid="cit12">12</xref>]);</p><p>– респираторные симптомы – одышка, особенно при физической нагрузке, персистирующий кашель, чувство стеснения в грудной клетке [<xref ref-type="bibr" rid="cit3">3</xref>];</p><p>– нейрокогнитивные симптомы – проблемы с концентрацией внимания («мозговой туман»), ухудшение памяти, замедление мышления, трудности при выполнении нескольких задач одновременно [13–16];</p><p>– кардиоваскулярные симптомы – ортостатическая непереносимость, учащенное сердцебиение, постуральная ортостатическая тахикардия (англ. postural orthostatic tachycardia syndrome, POTS), боли в грудной клетке, тахикардия покоя [17–20];</p><p>– неврологические симптомы – головные боли, головокружение, периферические нейропатии, нарушения сна [<xref ref-type="bibr" rid="cit3">3</xref>][<xref ref-type="bibr" rid="cit21">21</xref>];</p><p>– психоэмоциональные нарушения – тревога, депрессия, посттравматическое стрессовое расстройство, а также панические атаки и суицидальные мысли [<xref ref-type="bibr" rid="cit3">3</xref>][<xref ref-type="bibr" rid="cit21">21</xref>].</p><p>Согласно метаанализу, включившему 41 исследование с общим числом участников 860 783 человека, факторами риска развития ПКС являются: возраст, женский пол, повышенный индекс массы тела, курение, сопутствующие заболевания (тревога и/или депрессия, астма, хроническая болезнь почек, хроническая обструктивная болезнь легких, диабет, иммуносупрессия, ишемическая болезнь сердца), а также отсутствие вакцинации против COVID-19 [<xref ref-type="bibr" rid="cit20">20</xref>].</p><p>Гетерогенность клинических проявлений и отсутствие единого патогномоничного признака усложняют диагностику ПКС, которая требует исключения других заболеваний. Многие симптомы являются неспецифическими и могут наблюдаться при иных состояниях [<xref ref-type="bibr" rid="cit22">22</xref>][<xref ref-type="bibr" rid="cit23">23</xref>], включая последствия интенсивной терапии, поствирусные синдромы другой этиологии, а также первичные психические расстройства [<xref ref-type="bibr" rid="cit3">3</xref>]. В связи с этим детальный сбор анамнеза, включая хронологию развития симптомов, и использование валидизированных опросников, специфичных для данного состояния (например, модифицированная шкала C19-YRSm1), имеют критическое значение для постановки диагноза и установления доверительных отношений с пациентом [<xref ref-type="bibr" rid="cit3">3</xref>][<xref ref-type="bibr" rid="cit24">24</xref>].</p><p>Своевременная и адекватная реабилитация больных с ПКС играет первостепенную роль в восстановлении функциональной активности, улучшении качества жизни, возвращении к трудовой деятельности и снижении долгосрочного бремени болезни [<xref ref-type="bibr" rid="cit3">3</xref>][<xref ref-type="bibr" rid="cit12">12</xref>]. Исследования показывают, что пациенты, прошедшие реабилитацию, демонстрируют лучшие показатели качества жизни по сравнению с теми, кто не получал реабилитационных вмешательств [<xref ref-type="bibr" rid="cit25">25</xref>].</p><p>Принимая во внимание многогранность поражения, реабилитационные подходы должны быть мультидисциплинарными и персонализированными, учитывать ведущие симптомы и лежащие в их основе патофизио­логические механизмы [<xref ref-type="bibr" rid="cit4">4</xref>][<xref ref-type="bibr" rid="cit12">12</xref>]. Клинические исследования подчеркивают, что гетерогенность ПКС требует разработки персонализированных реабилитационных планов с учетом доминирующих симптомов – например, легочная реабилитация при респираторных последствиях, физическая реабилитация с применением принципов Pacing при выраженной утомляемости и PEM [<xref ref-type="bibr" rid="cit26">26</xref>], когнитивная реабилитация при «мозговом тумане», а также психологическая поддержка и эрготерапия при тревожно-депрессивных расстройствах и трудностях возвращения к повседневной активности [<xref ref-type="bibr" rid="cit12">12</xref>][<xref ref-type="bibr" rid="cit27">27</xref>].</p><p>Перспективной моделью является подход «излечимых признаков» (англ. treatable traits), адаптированный для реабилитации, – выделение ключевых функциональных нарушений (снижение толерантности к нагрузке, когнитивный дефицит, ортостатическая непереносимость) и подбор соответствующих реабилитационных вмешательств, направленных на коррекцию этих конкретных состояний [<xref ref-type="bibr" rid="cit28">28</xref>][<xref ref-type="bibr" rid="cit29">29</xref>]. Важно также учитывать социальные факторы (доступ к реабилитационным услугам, социально-экономический статус и психосоциальная поддержка), способные существенно влиять на траекторию восстановления пациента и его приверженность к реабилитационным программам [<xref ref-type="bibr" rid="cit30">30</xref>][<xref ref-type="bibr" rid="cit31">31</xref>]. Нередко встречаются повреждения внутренних органов и систем, к которым наиболее часто относятся пневмосклероз, повреждение сердечно-сосудистой, центральной нервной и иммунной систем [<xref ref-type="bibr" rid="cit4">4</xref>][<xref ref-type="bibr" rid="cit7">7</xref>].</p></sec><sec><title>ПАТОФИЗИОЛОГИЯ ПОСТКОВИДНОГО СИНДРОМА / POST-COVID SYNDROME PATHOPHYSIOLOGY</title><p>Понимание патофизиологических механизмов, лежащих в основе ПКС, является фундаментом для разработки эффективных реабилитационных стратегий (табл. 1). Современные исследования указывают на мультифакторную природу этого состояния, в которой переплетаются несколько ключевых механизмов [<xref ref-type="bibr" rid="cit3">3</xref>][<xref ref-type="bibr" rid="cit4">4</xref>][<xref ref-type="bibr" rid="cit32">32</xref>][<xref ref-type="bibr" rid="cit33">33</xref>]. Многообразие и взаимосвязь этих патофизиологических механизмов объясняют гетерогенность клинических проявлений ПКС и подчеркивают необходимость комплексного, мультисистемного подхода к реабилитации [<xref ref-type="bibr" rid="cit3">3</xref>][<xref ref-type="bibr" rid="cit4">4</xref>][<xref ref-type="bibr" rid="cit12">12</xref>].</p><table-wrap id="table-1"><caption><p>Таблица 1. Она не может быть сведена к единому протоколу, а требует учета ведущих симптомов, их выраженности и предполагаемых патофизиологических механизмов и должна быть направлена на восстановление функциональных резервов организма и качества жизни пациентов [<xref ref-type="bibr" rid="cit3">3</xref>][<xref ref-type="bibr" rid="cit12">12</xref>][<xref ref-type="bibr" rid="cit19">19</xref>][<xref ref-type="bibr" rid="cit35">35</xref>].</p><p>Реабилитация начинается с исключения острых осложнений (тромбоэмболия легочной артерии, миокардит, фиброз легких, эндокринные нарушения и др.) [<xref ref-type="bibr" rid="cit3">3</xref>][<xref ref-type="bibr" rid="cit19">19</xref>]. План разрабатывается совместно с пациентом, цели должны быть реалистичными и измеримыми [<xref ref-type="bibr" rid="cit3">3</xref>][<xref ref-type="bibr" rid="cit35">35</xref>][<xref ref-type="bibr" rid="cit36">36</xref>]. Интеграция принципов эргономики – человеко-факторной инженерии (англ. human factors engineering, HFE) позволяет создавать адаптивные программы, учитывающие индивидуальный темп и энергетические возможности пациента, что критически важно для предотвращения PEM [<xref ref-type="bibr" rid="cit12">12</xref>]. HFE-подход минимизирует когнитивную и физическую нагрузку, ориентируясь на взаимодействие пациента с окружением, а не на жесткие нормативы [<xref ref-type="bibr" rid="cit12">12</xref>].</p><p>Обучение больных методам самоконтроля является крае­угольным камнем долгосрочного ведения ПКС [<xref ref-type="bibr" rid="cit3">3</xref>]. Базовые элементы включают образовательные программы, помогающие пациентам понять природу их заболевания, распознавать ранние признаки ухудшения, а также ставить реалистичные цели и вести дневники симптомов [<xref ref-type="bibr" rid="cit3">3</xref>][<xref ref-type="bibr" rid="cit12">12</xref>].</p></sec><sec><title>Коррекция физической активности / Correction of physical activity</title><p>Физическая реабилитация является центральным компонентом восстановления, однако требует особой осторожности из-за риска PEM – патологической реакции на физическую, когнитивную или эмоциональную нагрузку, при которой симптомы значительно усиливаются через 12–72 ч после активности, и это ухудшение может длиться днями или неделями [<xref ref-type="bibr" rid="cit3">3</xref>][<xref ref-type="bibr" rid="cit12">12</xref>]. Данный феномен, хорошо известный при синдроме хронической усталости / миалгическом энцефаломиелите, часто наблюдается при ПКС [37–39].</p><p>В отличие от традиционных моделей, полагающихся на стандартизированные, прогрессивно усложняющиеся упражнения, физическая реабилитация при ПКС должна строиться на адаптивных подходах, учитывающих вариа­бельность симптомов и ставящих во главу угла безопасность пациента [<xref ref-type="bibr" rid="cit12">12</xref>]. Ключевые концепции такого подхода – Pacing2, «энергетический конверт» и микровыборные подходы (англ. micro-choice-based approaches), которые взаимно дополняют друг друга.</p><p>Основой физической реабилитации является тщательное дозирование активности на основе принципа Pacing, исключающее наращивание нагрузки «через усталость» [<xref ref-type="bibr" rid="cit3">3</xref>][<xref ref-type="bibr" rid="cit12">12</xref>][<xref ref-type="bibr" rid="cit26">26</xref>]. М. Parker et al. [<xref ref-type="bibr" rid="cit26">26</xref>] продемонстрировали, что 6-недельный структурированный протокол Pacing приводит к значительному снижению эпизодов PEM (с 3,4 до 1,1 в неделю; p&lt;0,001) и клинически значимому улучшению общего состояния здоровья по европейскому опроснику качества жизни EQ-5D-5L (с 51,4 до 60,6 балла; p&lt;0,001).</p><p>Протокол Pacing включает [<xref ref-type="bibr" rid="cit26">26</xref>]:</p><p>– определение индивидуального базового уровня активности, который не приводит к обострению симптомов;</p><p>– чередование активности и отдыха, причем отдых должен планироваться до наступления сильной усталости;</p><p>– постепенное увеличение нагрузки (не более чем на 10–20% в неделю) только при стабильном состоянии и отсутствии PEM;</p><p>– фракционирование нагрузки (разбивку крупных задач на несколько мелких этапов с периодами отдыха).</p><p>Ключевым элементом предотвращения рецидивов и поддержки устойчивого восстановления является обучение пациентов распознаванию и пребыванию в пределах своего индивидуального «энергетического конверта» – безопасного диапазона активности, не провоцирующего ухудшение состояния [<xref ref-type="bibr" rid="cit12">12</xref>]. Эта концепция задает персональные границы, внутри которых человек может функционировать без риска PEM. Внутри границ «энергетического конверта» необходима гибкость для принятия повседневных решений.</p><p>Микровыборные подходы позволяют пациентам принимать автономные решения об уровне активности в реальном времени, основываясь на текущем самочувствии, что минимизирует обострения симптомов и поддерживает чувство контроля [<xref ref-type="bibr" rid="cit12">12</xref>].</p><p>Таким образом, Pacing предоставляет структуру и правила, «энергетический конверт» определяет безопасные границы, а микровыборные стратегии дают инструменты для гибкого поведения внутри этих границ. Комплексное применение этих подходов составляет основу современной физической реабилитации пациентов с ПКС.</p></sec><sec><title>Респираторная реабилитация / Respiratory rehabilitation</title><p>У пациентов с персистирующими респираторными симптомами, такими как одышка и кашель, респираторная реабилитация играет важную роль. Однако, учитывая риск PEM, к ней также следует подходить с осторожностью.</p><p>Основные компоненты респираторной реабилитации включают:</p><p>– тренировку дыхательных мышц – использование пороговых тренажеров для укрепления диафрагмы и вспомогательных дыхательных мышц (у пациентов отмечено статистически значимое увеличение жизненной емкости легких с 78,27% до 85,36%; p=0,0006 [<xref ref-type="bibr" rid="cit40">40</xref>]);</p><p>– техники контроля дыхания – обучение больных методам управления одышкой (дыхание с сомкнутыми губами, диафрагмальное дыхание, «квадратное дыхание») имеет первостепенное значение, поскольку у пациентов с легким течением COVID-19 дискомфорт при дыхании часто обусловлен функциональными, а не органическими нарушениями [<xref ref-type="bibr" rid="cit3">3</xref>][<xref ref-type="bibr" rid="cit40">40</xref>];</p><p>– техники очищения дыхательных путей – активный цикл дыхательных техник, использование положительного давления на выдохе для мобилизации мокроты [<xref ref-type="bibr" rid="cit3">3</xref>];</p><p>– постуральный дренаж и позиционирование тела – использование положений тела, облегчающих дыхание и дренаж мокроты (в исследовании А.Ф. Беляева и др. показано, что у 98,8% пациентов выявляются локальные соматические дисфункции позвоночника, ребер и диафрагмы, что обосновывает применение позиционирования и дренажных техник [<xref ref-type="bibr" rid="cit39">39</xref>]).</p><p>Важно отметить, что у части больных с ПКС наблюдается паттерн нарушенного (дисфункционального) дыхания, клинически проявляющийся ощущением нехватки воздуха и невозможностью сделать полный вдох или выдох [<xref ref-type="bibr" rid="cit3">3</xref>][<xref ref-type="bibr" rid="cit41">41</xref>]. Одной из его форм является синдром гипервентиляции, описанный у пациентов после легкого течения COVID-19 [<xref ref-type="bibr" rid="cit4">4</xref>]. В таких случаях обучение контролируемому дыханию приобретает первостепенное значение [<xref ref-type="bibr" rid="cit3">3</xref>][<xref ref-type="bibr" rid="cit41">41</xref>][<xref ref-type="bibr" rid="cit42">42</xref>].</p></sec><sec><title>Когнитивная реабилитация / Cognitive rehabilitation</title><p>Когнитивная реабилитация направлена на улучшение таких функций, как внимание, память и исполнительные функции, и включает не только восстановительные, но и компенсаторные стратегии [<xref ref-type="bibr" rid="cit3">3</xref>].</p><p>Восстановительные стратегии (когнитивный тренинг) используют повторяющиеся упражнения для улучшения конкретных когнитивных функций, в т.ч. с применением компьютерных программ [<xref ref-type="bibr" rid="cit3">3</xref>]. Перспективным методом является тренинг на основе биологической обратной связи (БОС) по альфа-ритму на электрокардиограмме. В рандомизированном контролируемом исследовании А.Н. Черкасовой и др. установлено, что 12–15 сеансов БОС-тренинга приводят к значимому снижению личностной тревожности, психологического стресса, депрессии и астении, а также к улучшению параметров запоминания информации [<xref ref-type="bibr" rid="cit35">35</xref>].</p><p>Компенсаторные стратегии направлены на адаптацию к имеющимся когнитивным ограничениям и включают использование ежедневников, списков дел, напоминаний, упрощение задач, избегание многозадачности [<xref ref-type="bibr" rid="cit3">3</xref>][<xref ref-type="bibr" rid="cit42">42</xref>]. Интеграция принципов когнитивной эргономики, являющейся частью HFE, позволяет оптимизировать дизайн задач и условия окружающей среды для снижения когнитивной нагрузки и улучшения выполняемости [<xref ref-type="bibr" rid="cit12">12</xref>]. В частности, это организация рабочего пространства, использование вспомогательных технологий и метакогнитивный тренинг [<xref ref-type="bibr" rid="cit12">12</xref>].</p><p>Терапия когнитивных функций, индуцированная ограничением (англ. сonstraint-induced cognitive therapy, CICT) показала положительные результаты. В пилотном исследовании G. Uswatte et al. у пациентов с «мозговым туманом» CICT продемонстрировала значительные улучшения в выполнении повседневной деятельности (средний прирост 3,7 балла; p&lt;0,001; d=2,6), снижение симптомов и возвращение к работе у 80% участников (против 0% в контрольной группе) [<xref ref-type="bibr" rid="cit43">43</xref>][<xref ref-type="bibr" rid="cit44">44</xref>].</p><p>Рандомизированное исследование S. Oliver-Mas et al. показало, что сочетание транскраниальной стимуляции постоянным током (англ. transcranial direct current stimulation, tDCS) с когнитивным тренингом безопасно и эффективно снижает усталость и улучшает когнитивные функции [<xref ref-type="bibr" rid="cit45">45</xref>].</p><p>Протоколы должны учитывать степень нарушений, коморбидность и эмоциональный статус [<xref ref-type="bibr" rid="cit42">42</xref>][<xref ref-type="bibr" rid="cit46">46</xref>][<xref ref-type="bibr" rid="cit47">47</xref>].</p></sec><sec><title>Психологическая поддержка и психиатрическая помощь / Psychological support and psychiatric care</title><p>Психологическая поддержка – неотъемлемая часть реабилитации при ПКС в связи с высокой распространенностью тревоги, депрессии и посттравматического стрессового расстройства [<xref ref-type="bibr" rid="cit3">3</xref>]. Исследования показывают, что легкие когнитивные нарушения выявляются у 26,9% пациентов, а психоневрологические дисфункции широко распространены независимо от тяжести перенесенной инфекции [<xref ref-type="bibr" rid="cit5">5</xref>]. Больные испытывают значительное психоэмоциональное напряжение, требующее применения специализированных психотерапевтических техник [<xref ref-type="bibr" rid="cit46">46</xref>][<xref ref-type="bibr" rid="cit47">47</xref>].</p><p>Психообразование информирует о связи физических симптомов с психоэмоциональным состоянием, формирует адекватное отношение к болезни и повышает приверженность к лечению [<xref ref-type="bibr" rid="cit3">3</xref>]. В отечественной практике оно интегрируется в комплексные программы реабилитации как этап формирования мотивации и освоения стратегий совладания со стрессом [<xref ref-type="bibr" rid="cit48">48</xref>].</p><p>Когнитивно-поведенческая терапия (КПТ) эффективна для лечения тревоги, депрессии и адаптации к хроническому заболеванию [<xref ref-type="bibr" rid="cit3">3</xref>]. О.И. Одарущенко и др. показали статистически значимый регресс эмоциональных нарушений по Госпитальной шкале тревожности и депрессии (англ. Hospital Anxiety and Depression Scale, HADS) и шкале Спилбергера–Ханина (p&lt;0,05) при включении КПТ в комплексную реабилитацию. Ключевые направления: коррекция когнитивных искажений и дисфункциональных убеждений, обучение релаксации и формирование копинг-стратегий [<xref ref-type="bibr" rid="cit48">48</xref>].</p><p>Терапия принятия и ответственности (англ. acceptance and commitment therapy, ACT) помогает сосредоточиться на ценностях и значимой деятельности, несмотря на симптомы [<xref ref-type="bibr" rid="cit3">3</xref>]. В российской практике этот подход реализуется через формирование адаптивных копинг-стратегий и принятие ограничений, связанных с болезнью [<xref ref-type="bibr" rid="cit10">10</xref>][<xref ref-type="bibr" rid="cit49">49</xref>].</p><p>Релаксационные техники снижают уровень стресса и улучшают сон [<xref ref-type="bibr" rid="cit3">3</xref>]. Исследование О.И. Одарущенко и др. подтвердило эффективность релаксационных техник и аудио­визуальной стимуляции в снижении тревожности и повышении субъективного комфорта [<xref ref-type="bibr" rid="cit48">48</xref>].</p><p>Медикаментозная терапия (антидепрессанты, анксиолитики) показана при тяжелых расстройствах и требует консультации психиатра [<xref ref-type="bibr" rid="cit3">3</xref>]. В российской практике применяется патогенетическая терапия препаратами, влияющими на нейротрансмиссию (холина альфосцерат) и обладаю­щими анксиолитическим действием. Применение холина альфосцерата у пациентов с постковидными когнитивными расстройствами приводит к статистически значимому улучшению показателей по Монреальской когнитивной шкале (англ. Montreal Cognitive Assessment, MoCA) и снижению уровня тревоги/депрессии по HADS [<xref ref-type="bibr" rid="cit10">10</xref>][<xref ref-type="bibr" rid="cit50">50</xref>].</p><p>Крайне важно дифференцировать тревогу от вегетативной дисфункции (POTS), имитирующей симптомы тревоги, для исключения соматических причин перед началом психофармакотерапии [<xref ref-type="bibr" rid="cit3">3</xref>][<xref ref-type="bibr" rid="cit38">38</xref>].</p></sec><sec><title>Коррекция вегетативных нарушений / Correction of autonomic disorders</title><p>У больных с ортостатической непереносимостью и POTS ключевую позицию занимают реабилитационные стратегии, направленные на улучшение функционального статуса [<xref ref-type="bibr" rid="cit3">3</xref>][<xref ref-type="bibr" rid="cit4">4</xref>]. Базовые мероприятия являются первой линией терапии и должны предлагаться всем пациентам [<xref ref-type="bibr" rid="cit51">51</xref>][<xref ref-type="bibr" rid="cit52">52</xref>]:</p><p>– управление гидратацией и диетой – увеличение потребления жидкости (до 2–3 л/сут) и соли (10–12 г/сут при отсутствии противопоказаний) как терапия первой линии [<xref ref-type="bibr" rid="cit52">52</xref>][<xref ref-type="bibr" rid="cit53">53</xref>];</p><p>– физические контрмеры – сжатие кистей, скрещивание ног, приседания перед вставанием для улучшения венозного возврата, обучение медленному вставанию [<xref ref-type="bibr" rid="cit3">3</xref>][<xref ref-type="bibr" rid="cit52">52</xref>];</p><p>– компрессионная терапия – чулки (30–40 мм рт. ст.) и абдоминальный бандаж для снижения прироста частоты сердечных сокращений при вертикализации [<xref ref-type="bibr" rid="cit51">51</xref>][<xref ref-type="bibr" rid="cit53">53</xref>];</p><p>– эргономические рекомендации – сон с приподнятым изголовьем, избегание провоцирующих факторов (перегрев, длительное стояние, алкоголь, кофеин) [<xref ref-type="bibr" rid="cit51">51</xref>][<xref ref-type="bibr" rid="cit52">52</xref>].</p><p>Физическая реабилитация играет центральную роль. Аэробные нагрузки и тренировки с сопротивлением проводятся по индивидуализированным программам с постепенным увеличением интенсивности [<xref ref-type="bibr" rid="cit51">51</xref>]. Структурированные программы длительностью 3–6 мес улучшают гемодинамические показатели и переносимость вертикальной нагрузки [<xref ref-type="bibr" rid="cit54">54</xref>]. Тренировки проводятся под наблюдением с учетом риска PEM [<xref ref-type="bibr" rid="cit12">12</xref>].</p><p>Медикаментозная поддержка (ивабрадин, бета-блокаторы, флудрокортизон) рассматривается как дополнительный инструмент при недостаточной эффективности базовых мероприятий [<xref ref-type="bibr" rid="cit52">52</xref>]. Препараты временно облегчают симптомы, создавая условия для более успешной реабилитации [<xref ref-type="bibr" rid="cit17">17</xref>]. Фармакотерапия не заменяет, а дополняет реабилитацию.</p><p>Реабилитационная программа должна быть строго индивидуализирована и строиться совместно с пациентом с учетом его функциональных возможностей и целей восстановления [<xref ref-type="bibr" rid="cit3">3</xref>][<xref ref-type="bibr" rid="cit12">12</xref>].</p></sec><sec><title>Телереабилитация и цифровые технологии / Telerehabilitation and digital technologies</title><p>Телереабилитация как часть информационно-коммуникационных технологий стала важным инструментом для обеспечения непрерывности и доступности реабилитационной помощи при ПКС [<xref ref-type="bibr" rid="cit12">12</xref>]. Обзор 2024 г. (10 исследований, 749 участников) подтверждает эффективность дистанционного проведения дыхательных упражнений, что улучшает функциональные показатели, уменьшает одышку и повышает общее благополучие пациентов [<xref ref-type="bibr" rid="cit55">55</xref>]. Дистанционный формат особенно ценен для людей с ограниченной мобильностью и для педиатрической популяции [<xref ref-type="bibr" rid="cit12">12</xref>][<xref ref-type="bibr" rid="cit56">56</xref>][<xref ref-type="bibr" rid="cit57">57</xref>].</p><p>Исследование C.Y. Lai et al. с участием 182 пациентов показало, что 12-недельная программа с еженедельным дистанционным мониторингом улучшает физическую активность (p&lt;0,001), самоэффективность (p&lt;0,001) и качество сна (p=0,012) [<xref ref-type="bibr" rid="cit57">57</xref>]. S. Cerfoglio et al. продемонстрировали улучшение двигательных и дыхательных функций после 3-недельной домашней программы (увеличение дистанции ходьбы на 18,3%) [<xref ref-type="bibr" rid="cit58">58</xref>]. N.C. Tura et al. показали, что 8-недельная синхронная телереабилитация (под наблюдением специалиста) превосходит асинхронную по приросту силы нижних конечностей (между группами p=0,02); внутри синхронной группы достоверно снижались также одышка (p=0,02) и утомляемость (p&lt;0,01) [<xref ref-type="bibr" rid="cit59">59</xref>]. M.J. Estebanez-Pérez et al. подтвердили эффективность 4-недельной индивидуализированной цифровой программы с улучшением функциональных возможностей (p&lt;0,05) [<xref ref-type="bibr" rid="cit60">60</xref>].</p><p>Основные проблемы включают равенство доступа, удобство использования и безопасность данных [<xref ref-type="bibr" rid="cit12">12</xref>]. Протокол RehabCOVID подчеркивает необходимость тщательного отбора пациентов (исключение лиц с моторными/сенсорными нарушениями) [<xref ref-type="bibr" rid="cit61">61</xref>]. Z. Yasacı et al. в рандомизированном клиническом исследовании показали, что 6-недельная супервизированная телереабилитация значимо уменьшает одышку, боль и улучшает функциональную способность у пациентов с ПКС по сравнению с неконтролируемыми домашними упражнениями [<xref ref-type="bibr" rid="cit62">62</xref>].</p><p>Интеграция цифровых инструментов на основе искусственного интеллекта может снизить частоту повторных госпитализаций до 50% [<xref ref-type="bibr" rid="cit12">12</xref>][<xref ref-type="bibr" rid="cit63">63</xref>].</p></sec><sec><title>Профессиональная реабилитация и возвращение к труду / Vocational rehabilitation and return to work</title><p>Поддержка безопасного и устойчивого возвращения к работе является центральным, но сложным аспектом реабилитации при ПКС [<xref ref-type="bibr" rid="cit12">12</xref>]. Метаанализ M. Ottiger et al., включивший 19 исследований с участием 21 155 пациентов, перенесших COVID-19, показал, что после ≥12 нед к работе вернулись лишь 60,9%, что подчеркивает необходимость комплексных реабилитационных программ [<xref ref-type="bibr" rid="cit64">64</xref>].</p><p>Принципы поэтапного возвращения</p><p>Традиционные линейные модели противоречат эпизодическому течению ПКС [<xref ref-type="bibr" rid="cit12">12</xref>]. Клинические рекомендации C.R. Rayner et al. предлагают гибкий подход: постепенное увеличение нагрузки, адаптация графика, возможность удаленной работы. План должен разрабатываться совместно с работодателем, сотрудником и реабилитационной командой начиная с неполного дня с регулярным пересмотром [<xref ref-type="bibr" rid="cit65">65</xref>].</p><p>Роль эрготерапии</p><p>Эрготерапевты оценивают функциональные способности, разрабатывают адаптационные мероприятия и вспомогательные средства [<xref ref-type="bibr" rid="cit12">12</xref>]. Систематический обзор С. von Zweck et al. подтверждает эффективность эрготерапевтических вмешательств (когнитивная реабилитация, управление утомляемостью) для улучшения профессионального функционирования [<xref ref-type="bibr" rid="cit66">66</xref>]. Цифровая программа онлайн-эрготерапии также показала улучшение качества жизни и когнитивных функций [<xref ref-type="bibr" rid="cit67">67</xref>].</p><p>Поддержка на рабочем месте</p><p>C.R. Rayner et al. детализируют меры в зависимости от симптоматики [<xref ref-type="bibr" rid="cit65">65</xref>]:</p><p>– физические симптомы (утомляемость, PEM, ортостатическая непереносимость) – поэтапное возвращение, регулируемая нагрузка, сидячие перерывы, гибкий график или удаленная работа;</p><p>– когнитивные нарушения («мозговой туман») – гибкий график, дробление задач, поэтапное возвращение с более простых заданий, использование цифровых инструментов;</p><p>– психические нарушения (тревога, депрессия) – гибкий график, удаленная работа, дополнительное время, модификация заданий для снижения стресса.</p><p>Цифровые инструменты</p><p>Носимые устройства и мониторинг на основе искусственного интеллекта помогают отслеживать симптомы и корректировать планы [<xref ref-type="bibr" rid="cit12">12</xref>]. Для пациентов с PEM эффективны индивидуализированные программы упражнений с отслеживанием «энергетического конверта» [<xref ref-type="bibr" rid="cit68">68</xref>].</p></sec><sec><title>Нутритивная поддержка / Nutritional support</title><p>Нарушения питания могут усугублять астению и мышечную слабость. Оценка нутритивного статуса и консультация диетолога рекомендуются пациентам со значительной потерей веса, сниженным аппетитом или признаками недостаточности питания. Диета должна быть сбалансированной, богатой белком для восстановления мышечной массы, а также витаминами и микроэлементами. Специфических диет или добавок, доказавших свою эффективность при ПКС, в настоящее время не существует, хотя исследования в этой области продолжаются [<xref ref-type="bibr" rid="cit4">4</xref>].</p></sec><sec><title>ПЕРСПЕКТИВЫ / PROSPECTS</title><p>ПКС представляет собой сложное мультисистемное состояние, патогенез которого включает персистенцию вируса, эндотелиальную дисфункцию, иммунную дисрегуляцию, вегетативные и митохондриальные нарушения [<xref ref-type="bibr" rid="cit3">3</xref>][<xref ref-type="bibr" rid="cit4">4</xref>][<xref ref-type="bibr" rid="cit7">7</xref>]. Гетерогенность клинических проявлений требует персонализированного подхода к реабилитации, основанного на тщательной оценке ведущих симптомов и функциональных ограничений каждого пациента.</p><p>Несмотря на потребность в реабилитации, доказательная база эффективности различных вмешательств все еще формируется. Многие рекомендации основаны на экспертных мнениях или экстраполяции данных из других областей [<xref ref-type="bibr" rid="cit3">3</xref>][<xref ref-type="bibr" rid="cit12">12</xref>]. Систематические обзоры показывают, что мультидисциплинарные амбулаторные программы улучшают физическую функцию, снижают утомляемость и тревогу, причем программы, сочетающие физические и психологические компоненты, дают более устойчивые результаты [<xref ref-type="bibr" rid="cit31">31</xref>][<xref ref-type="bibr" rid="cit32">32</xref>]. Подчеркивается важность индиви­дуализированного подхода к нагрузкам [<xref ref-type="bibr" rid="cit19">19</xref>].</p><p>Ключевые ограничения существующих исследований – короткие сроки наблюдения, отсутствие стандартизированных протоколов и гетерогенность исходов [<xref ref-type="bibr" rid="cit12">12</xref>][<xref ref-type="bibr" rid="cit31">31</xref>][<xref ref-type="bibr" rid="cit32">32</xref>]. Это определяет необходимость хорошо спланированных клинических исследований с использованием стандартизированных, ориентированных на пациента исходов.</p></sec><sec><title>ЗАКЛЮЧЕНИЕ / CONCLUSION</title><p>ПКС – это гетерогенное мультисистемное состояние, патогенез которого обусловлен взаимодействием персистенции вируса, иммунной дисрегуляции, эндотелиальной дисфункции, расстройств вегетативной нервной системы и митохондриальных нарушений. Именно это определяет необходимость мультидисциплинарного и персонализированного подхода к реабилитации, в рамках которого программа выстраивается вокруг ведущих симптомов и функциональных ограничений конкретного пациента.</p><p>Современная стратегия реабилитации основывается на интеграции принципов HFE, концепции «излечимых признаков» и стратегии Pacing с учетом риска PEM. Ключевыми элементами являются обучение пациента самоконтролю в рамках «энергетического конверта», поэтапное возвращение к физической и профессиональной активности, респираторная и когнитивная реабилитация, психологическая помощь, коррекция вегетативных нарушений, нутритивная поддержка и использование телереабилитационных и цифровых инструментов для расширения доступности реабилитационной помощи.</p><p>Доказательная база эффективности отдельных вмешательств продолжает формироваться, и приоритетом будущих исследований должны стать хорошо спланированные рандомизированные контролируемые исследования с длительным наблюдением и стандартизированными, ориентированными на пациента исходами.</p><p>1.
