Диагностика, лечение и реабилитация новорожденных с повреждением тканей центральной нервной системы (ЦНС) являются актуальной проблемой современного медицинского сообщества. Ранняя диагностика гипоксически-ишемических нарушений ЦНС – ключевой элемент в стратегии ведения новорожденных с выявленной асфиксией. Поиск чувствительных и специфичных диагностических инструментов позволит улучшить прогноз исхода заболевания и эффективность проводимой реабилитации.</p></sec><sec><title>Цель</title><p>Цель: выделить перспективные сывороточные биомаркеры повреждения тканей головного мозга для скрининга и мониторинга состояния новорожденных.</p></sec><sec><title>Материал и методы</title><p>Материал и методы. Поиск публикаций проведен автоматически в научных базах данных PubMed/MEDLINE, Google Scholar, ScienceDirect и электронной библиотеке eLibrary, а также вручную в поисковых системах Яндекс, Google. Последующий отбор исследований выполнен с учетом рекомендаций PRISMA. Из обнаруженных 3778 публикаций в обзор включены 52.</p></sec><sec><title>Результаты</title><p>Результаты. Представлены перспективные биомаркеры повреждения головного мозга, определяемые в сыворотке крови у новорожденных: дипептидилпептидаза-4, ряд цитокинов, нейронспецифичная енолаза (НСЕ), бутилкарнитин, кальций-связывающий белок B S100 (S100B), F2-изопростаны, ядросодержащие эритроциты, а также некоторые другие биомаркеры, изменение уровня которых коррелировало с выраженностью нарушения тканей ЦНС.</p></sec><sec><title>Заключение</title><p>Заключение. Дисфункция гематоэнцефалического барьера вызывает появление в сыворотке крови высокоспецифичных белков повреждения нейронов и глии, в частности S100B и НСЕ, которые можно определить как биомаркеры. Необходимо расширять доказательную базу и валидацию использования указанных биомаркеров с целью последующего внедрения в клинические рекомендаций по ведению пациентов с повреждением тканей ЦНС в раннем постнатальном периоде для своевременной диагностики и коррекции реабилитационных мероприятий.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Background</title><p>Background. Diagnostics, treatment and rehabilitation of newborns with injury of the central nervous system (CNS) tissues are one of the pressing problems of contemporary medical community. Early diagnostics of hypoxic-ischemic damage to CNS tissues is a key element in the strategy of supporting newborns with asphyxia. The search for sensitive and specific diagnostic tools will improve the prognosis of disease outcome and the effectiveness of rehabilitation.</p></sec><sec><title>Objective</title><p>Objective: to identify promising serum biomarkers of brain tissue damage for screening and monitoring in neonates.</p></sec><sec><title>Material and methods</title><p>Material and methods. The search for publications was carried out automatically in scientific databases (PubMed/MEDLINE, Google Scholar, ScienceDirect) and electronic library (eLibrary) as well as manually in search engines (Yandex, Google). The subsequent selection of articles was carried out according to PRISMA recommendations. Of 3778 publications found, 52 were included in the review.</p></sec><sec><title>Results</title><p>Results. The promising biomarkers of brain injury determined in the blood serum of newborns were detected: dipeptidyl peptidase 4, a number of cytokines, neuron-specific enolase (NSE), butylcarnitine, calcium-binding protein B S100 (S100B), F2-isoprostanes, nucleated erythrocytes, and some other biomarkers, the change in the level of which correlated with the severity of CNS tissue damage.</p></sec><sec><title>Conclusion</title><p>Conclusion. Blood-brain barrier dysfunction causes the appearance of highly specific proteins of neuronal and glial injury, in particular S100B and NSE, in blood serum, which can be determined as biomarkers.
